We are an out-of-network, cash pay physical therapy and wellness facility. You may be able to have out-of-network benefits paid for depending on your insurance. We offer self billing assistance but do not guarantee reimbursement. All of QLife’s clients have opted to forgo the insurance hassles in order to receive to highest quality care and attention they deserve. Physical Therapy:

  • Initial Physical Therapy Evaluation - $175

  • 1 Hour Physical Therapy Visit - $150

  • 30 Min. Physical Therapy Visit - $80

  • 1 Hour Telehealth Physical Therapy Visit - $140

  • 30 Min. Telehealth Physical Therapy Visit - $75

  • Packages and Installment Plans available - up to $100 in savings

Wellness/Fitness:

  • Initial Fitness Consultation - $150

  • 1 Hour Fitness Visit - $100

  • 30 Min. Fitness Visit - $55

  • Memberships:

    • Premier- 12 1 hour sessions, $1140/month

    • Dedicated- 8 1 hour sessions, $760/month

    • Commit- 4 1 hour sessions, $380/month

    • Premier Lite- 12 half hour sessions, $630/month

    • Dedicated Lite- 8 half hour sessions, $420/month

    • Commit Lite- 4 half hour sessions, $220/month

Group Classes

  • Up to 3 attendees max for $150 total- Bring your friends and family
