Lower back pain is one of the most common reasons someone books a PT appointment, and also one of the most over-treated conditions in healthcare — lots of interventions, not all of them backed by much evidence.
Almost anything can make you temporarily feel better, but is that what you really want, temporary relief?
What holds up: staying active rather than resting completely, which sounds backwards when something hurts, but prolonged rest is associated with worse outcomes for most non-specific back pain. Targeted strengthening — particularly progressive core and hip musculature — consistently outperforms passive treatments like heat or ultrasound alone. The combination of exercises and manual therapy has the best evidence. You can’t just passively recieve treatment, you have to be a part of it.
For sciatica specifically, the picture's more nuanced. Directional exercises — movements that centralize the pain back toward the spine rather than out toward the leg — are often a useful early signal for what's actually going to help, and a good therapist tests for this directly instead of guessing.
What doesn't help much on its own: imaging for most non-specific back pain (an MRI often shows "abnormalities" present in pain-free people too, which just creates unnecessary alarm), and passive modalities used as the primary treatment instead of a supplement to actual exercise.
